Spring Track Pre-Season Info

Spring Track Pre-Season Forms

The forms you need to submit will depend completely on whether you have done cross country and/or winter track in 2017-18. We are trying to minimize paperwork, and as such, if you have done one or two seasons this year, you may have less to turn in.

Please understand that we will maintain our basic team policies from one season to the next, and by acknowledging the rules for one season, you are agreeing to comply with them in the next.

1) Provide me with your personal data for my database, using the APPROPRIATE online survey, listed below. Fill this out completely. It will provide me with all of the personal info I will need to enter you in our database, build our email list and put you on our roster. Please note that I am using two different surveys: one for athletes returning from 2017-18 Winter Track or 2017 Cross Country, and a separate one for athletes who did not run track this winter (even if you ran track last year).

2018 Spring Track Returning Athlete Survey (This is ONLY for those athletes who ran Cross Country 2017 or Winter Track 2017-18)

Spring Track New Athlete Survey (This is for all athletes who are either new this year, or who ran spring track last year, but not Cross Country or Winter Track this year)

2) Below, please have your parent fill out the link labeled: Spring Track Parent Info. This will give me emergency contact info, and an email address which will allow us to keep you informed regularly.

2018 SPIRIT PACK FORM (Due back to Coach Franks by February 9th) (MUST BE SUBMITTED EVEN IF NOT PURCHASING ANYTHING)

You will need, at minimum, to purchase the following items:

PLEASE NOTE: TEAM SHIRT, JERSEY and Shorts (BOYS) are the same as last year and cross country.

  • i. Team Shirt (Short Sleeved White) (to be worn at meets) (New in Spring 2016)
  • iii. Jersey
  • iv. Shorts(Running Shorts or Spandex-style(Boys) or BoyShorts (Girls) ; Mesh Shorts for Throwers Only

NOTE: The Spirit Pack ordering is now done online, through Cassels' Online store (http://stores.inksoft.com/Chantilly_Spring_Track/All-Products/-1). We will put in our first order on Thursday, February 22nd.. If you are certain to make the team, please consider ordering early, to ensure that you get your uniform before the first meet.

REFUND POLICY- For any athlete who tries out for track, but does not make the team, we will agree to refund that athlete for the REQUIRED items purchased (not sweats, warmups, etc). If you place an order, but decide not to tryout, then we will not be able to give you a refund, although we will certainly provide you with your uniform when it is delivered.

4. Emergency-Care Card (for everyone, unless they did cross-country or winter track)

5. VHSL Physical Form- This MUST be conducted after May 1st, 2017, and be submitted prior to Tryouts. This means that it must a be a new physical for this year, not simply taking it to you doctor and signing off on a physical done sometime last year, prior to May 1st. I realize that this may be difficult for some people due to insurance limitations, which may only allow one physical per year, but we have no flexibilty on this. SCHEDULE THIS ASAP. If need be, there is a walk-in clinic in Greenbriar. Also, all student-athletes must have insurance, and listed on their physical. If need be, there is an inexpensive student policy available. You can download the form here, and turn it into the activities office. (Student Athletic Accident Insurance)

6. Concussion Education Program (Athlete & Parent)- As of July 1st, 2011, FCPS is requiring that each athlete and ONE parent must complete a short Concussion Education program online, good for one school year. The proogram is found at the preceding link. This can be done anytime during the school year, but must be done prior to tryouts. Nothing needs to be printed, as it will be recorded by Mr. Castor, and a report will be submitted to Coach G.

7. FCPS Extra-Curricular Policy Form (this is required each season). This is Fairfax County Public School system policy (not Chantilly HS), as it pertains to your rights and responsibilities regarding substance abuse and/or any other actions which may be considered illegal or otherwise detrimental to your participation in FCPS team sports.

8. Team Bible- Return the last page, including ALL initials/signatures from athlete and parents. This contains all of our team rules and regulations, as well as information about tryouts, our schedule, and our website.

9.2018 Spring Track Forms List- Please check to see if you owe any forms still. If a certain column is colored, you have not turned in that form. If there is a notation, you are missing part of the form (ie: Insurance Info for the Physical; or a parent signature somewhere). The Athlete Info, & Parent Info, are online surveys, and can be accessed above. Please fill these out completely. If you believe that this is in error, bring it to my attention, but I would still prefer that you simply print out the form again, and re-sign it. I will admit to any error I make, but when it comes down to it, if your form is missing, it is missing. Please get them in ASAP.
