If you know of anyone that would benefit the team by trying out please pass on this information.

We will be having two interest meetings before the season. The first will be this Thursday, January 23rd at 3:10 in the cafeteria. The second will be Thursday February 20th at 3:10 in the cafeteria to collect the completed and signed Emergency Care Cards and the Participation Policies and to answer any questions.

The Physical Form and the Concussion Form get turned in to either Ms. Bishop or Mr. Boss in the trainer’s room. They record them for you and run the report that we MUST follow. The Emergency Care Card and Participation Policy will be collected by Coach Sciabica at the second interest meeting. If your forms aren’t turned in and recorded PRIOR to the first day of try-outs you cannot participate on the first day! DO NOT wait to schedule an appointment for a physical, if you do you may find that your doctor can’t get you in before try-outs thus you cannot try-out until it is completed, recorded, and Ms. Bishop or Mr. Boss send us the report!

Below are links to the documents or links to their location. EVERYTHING!  MUST be completed BEFORE the first day of Try-Outs! You cannot step on the courts for try-outs if these are not completed, turned in, and recorded. We cannot check the day of try-outs with Ms. Bishop or Mr. Boss to see if you have turned in your Physical or Concussion form.

The forms that need to be completed, signed by you and a parent, and turned in PRIOR to trying out are:

  1. Physical Exam Form – Must be completed after May 1st, 2019 or later.
  2. FCPS Concussion Info please click HERE. After the Concussion Training is completed, go to page 48 of the Concussion Document, Click Print on that page, complete, and turn in with your Physical to Ms. Bishop or Mr. Boss! Please turn both in at the same time!
  3. Emergency Care Card
  4. Participation Policy
