Chantilly High School Physical Night
THe Chantilly High School Athletic Training Program will be offering sports physicals at Chantilly High School on June17th, 2013. This physical will be good for all sports and out of season practices (green days, yellow days, weight room workouts) for the 2013-2014 school year. The cost of the physical is $50 when you register online. VHSL Physical Form
This form must be completed and signed by a Physician, dated after May 5, 2012, and turned in to the Athletic Training Staff before a player is eligible to tryout or participate in any off-season conditioning.
Emergency Care Card
Please fill out 2 copies of this form and turn in to the coaching staff. An ECC must be completed for each sport a player plays
Extracurricular Participation Policy
The EPP must be read and signed by both player and parent before a player is able to tryout
Concussion Education

Insurance
