Concussion Education 20-21
The concussion education for the 2020-2021 School Year has been updated however will still remain a paper version. Open the attachment and review the PowerPoint. Once you finish the presentation the student-athlete and parent will sign the last page and submit it. Due to Covid19 you can email the signed last page to kabishop@fcps.edu
VHSL Physical Form
Attached is the most updated VHSL form. Page 1,2,4 must be filled out by a parent and student-athlete and page 3 must be completed at a dr.s office. Due to Covid please email all completed physicals to kabishop@fcps.edu.
