Prior to tryouts this form must be completed by Doctor, parent and player.
Emergency Care Card
Please fill out 2 copies of this form. This is necessary for basketball specifically as we do not share forms between teams.
Booster Membership
As the Boosters do much to support all programs including the Girls Basketball program, we invite you to join the Boosters and support athletics at Chantilly High School.
Extra Curricular Participation Policy
This is required to be filled out by all players prior to tryouts.
