Players should read and be aware of these expectations when attending tryouts.
VHSL Physical Form 2017
Physical Form - revised 2017. All players must have this form completed, signed by a doctor and on file before they may participate.
Extra Curricular Participation Policy
All players and parents are required to sign and return this form prior to participation
Emergency Care Card
All players must complete this form and return to their basketball coach.
Concussion Education
Concussion Education Required training for every parent and student athlete in FCPS. All student athletes and a parent must complete a concussion education program separately every school year according to Virginia law. Videos are provided at www.fcps.edu/sports. Please copy and paste the url into your browser and complete the program.
Field Trip Permission Form - Season
Players should complete this form prior to the season to ensure transportation to games and practices if transportation from the county is unavailable.
